Our trained staff evaluates each individual’s potential care needs at the time he or she applies at The Meadows. In collaboration with the resident, the resident’s legal representative or family (if agreed to by the resident), and a licensed nurse, the appropriate Level of Care with be determined through completion of a Negotiated Service Agreement. This agreement is reviewed annually, revised if necessary, and revised more frequently if the resident’s condition changes. The change may be a result of improvement or deterioration in the resident’s condition.
Level I
Apartment of choice Three home-style meals per day Utilities except telephone Cable TV Emergency call system Weekly housekeeping service Weekly laundry of bed & bath linens Interior & exterior maintenance Activity & social programs Laundry facilities On-site hair salon 24-hour nursing service Daily well-being checks Regular trash removal
Level II (additional $120 per month)
Includes all services provided in Level I. Minimal assist (cueing and encouragement) with two or fewer of the following: Bathing two times weekly Mealtime needs Medication management (filling of medication minders) & administration Ordering of medication Dressing Personal laundry Wound management Assistance scheduling appointments Personal shopping service Transportation to scheduled medical appointments Assist with dressing & grooming Assist with application & removal of TED hose
Level III (additional $240 per month)
Includes all services provided in Level I. Minimal assist with three or more of the services listed in Level II Assistance with the following as needed: Daily bed making Incontinence management program Assist of one-person for transfer Assist of one-person for ambulation Assist with bathing more than twice weekly
Level IV (additional $360 per month)
