Clinic visits without leaving home!
The best option for clinic appointments is always an in-person visit with your provider. However, we now offer online appointments, allowing you to interact with your provider via video link. Telehealth provides healthcare services from the comfort of their own homes. Please note, not all visits are suitable for telehealth.
1. Call your convenient CHS clinic.
2. Provide an email address to which the clinic will send an invitation to join StarLeaf, the platform used for telehealth visits.3. The StarLeaf invitation will come from chstelemedicine.coffeyhealth.org. Accept the invitation and ensure the StarLeaf application is downloaded on your device.
4. Visit www.coffeyhealth.org/Clinics/telehealth/ and complete the three forms. Please disregard the icon that states “Click to Send to CHS.” Instead, save the forms to your device and email them to email@example.com. We apologize for this inconvenience.
5. In advance of your scheduled appointment, click “join this StarLeaf meeting” to ensure the application is functioning correctly. For your appointment, launch the StarLeaf application, enter your name, and click “Join now.” 6. 15 minutes prior to your appointment, a receptionist will call to verify information, take credit card payment, and verify that the proper forms have been submitted.

Yes! As long as your diagnosis doesn't require further in-person
examination, your medical provider can simply electronically prescribe your medication and send it directly to the nearest pharmacy of your choice.
